- One adult will be permitted per patient
- No siblings will be permitted unless they have an appointment at the same time
- COVID screening forms on our website need to be completed by every person presenting to the office (parents and children) prior to coming to the office
- Patients and parents must wear a face covering unless they are under the age of 2 as per state law
- Families will check in by phone when they arrive at the office and wait in their car.
- When we are ready to see your child, the front desk staff will call you and let you know
- Your temperature will be checked. Please advise that anyone with a temperature of 100.4 or above cannot be seen
- Patients must keep masks on until they have been seated in their assigned operatories – Parents must keep masks on for the duration of time in the office
